Soft tissue injuries including whiplash, sprains, strains, cuts, bruises or partial muscle tear.

Injuries include severe brain injury, blindness, amputation, or paraplegia.

Form to Complete

Complete and sign the Accident Benefits Application (OCF-1).

Treatment plan

Your treatment provider will prepare a treatment confirmation form for you to sign.

Treatment and resolution with medical and rehabilitation benefits are limited to $3,500.

Your treatment provider will prepare a treatment plan for you to review and sign.

For catastrophic injuries, you will also need to complete additional forms.
